   Eye Care for Kids Foundation is grateful to have over 100 provider doctors on board throughout the Greater Houston Area. Our providers are essential to the success of this organization.  We always welcome new providers!

   Please consider joining our cause.  We'll  require your license number, tax id, and a signed program agreement form. You can call our office, and we will be glad to provide you with the Program Overview and Agreement.

Why join?
    Every year thousands of students in the Greater Houston Area go through school without corrected vision. As a result, these students are not able to participate and learn at their full potential. Eye Care for Kids Foundation works with all 54 School Districts to help those students who fall through the health care crack because of a lack of insurance.

    Becoming a "Collaborator in Caring", gives you an opportunity to help out your local community. The students referred to you, will be from schools within your vicinity.

    Doctors are reimbursed $79 for each student served, the Foundation makes payment on every 15th and 30th of the month and making claims is fast and easy.

Dear Providers, Please be advised that Eye Care for Kids will be closing its books for the 2008-2009 Fiscal Year on July 24, please make sure all your claims have been submitted prior to this date. You must sign and submitt the following form to our office, so that we may continue making referrals to your office for the 2009-2010 school year. Thank you for your cooperation!
You must submit copies of vouchers and RX for high prescription polycarbonate lenses.
