                       ELIGIBILITY

REFERRALS

To be eligible for Eye Care for Kids Foundation services, a child must:

i)  be referred by their school nurse.

ii) attend a school that is in one of the 56 school districts we serve (See about us.)

VISUAL ACUITY

Students who have a visual acuity of 20/40 or higher in either eye are

eligible, but they must also meet the financial criteria outlined below.

FINANCIAL CRITERIA

School nurses send home an Eye Care for Kids application for a student's

guardian or parent to fill out.  Eligible students:

i)  do not have health insurance, have insurance but not vision coverage, are not covered by CHIP, VSP, or Medicaid.

ii) have parents whose combined income does not exceed 150% of the

Federal Poverty  Guideline.
